All teams traveling from out of state need to complete a
permission to travel form, if required by your home state. Rules vary by state, and it is your responsibility to determine if this form is necessary.
The form is supplied by the U.S. youth soccer
association and must be printed and filled out through your state association.
On the form when it asks for tournament director,
