Kicks 4 Kids requires coaches to carry signed medical release forms for each participant.  It is not necessary that you use our specific form.  If you have medical release forms for your league, they will be sufficient.

Coaches will be required to show the release forms at the registration meeting.  Players without release forms will not be permitted to play.
