T-Shirt Pre-Order Form  Deadline for T-shirt Pre-Orders is May 12, 2006.

Permission to Travel Form: All teams traveling from out of state need to complete a permission to travel form, if required by your home state.  Rules vary per state, and it is your responsibility to determine if this form is necessary.  The form is supplied by the U.S. youth soccer association and must be printed and filled out through your state association.  Please make sure your team complies with the U.S. Youth Soccer Travel Policy.  US Club soccer teams do not need to apply for permission to travel.

Permission to Host Form: Out of state teams will need to print this form to send along with your Permission to Travel Form.

Medical Release Form:  All players need to have a medical release form completed before participating in the tournament.  This is a sample release form that can be printed and copied.  If your team already has medical release forms, they will be acceptable.
